A short preliminary telephone screening will be done when you are contacted by our office

Patient-information forms will be sent to you via mail or e-mail with instructions to send your current x-rays (taken within six months or less) and any other documentation

If you don’t have current x-rays re-taken, inform your doctor to order these views:

a)	Standing AP views of both ankles
b)	AP, lateral, flexion, extension, oblique and mortise views of the affected ankle
